Election Forms
Voter Registration is open to all Duchesne County residents that are:
- Citizens of the United States
- Residents of Utah for at least 30 days prior to Election Day
- Be at least 18 years of age on or before the day of the General Election
- 16-17 years old may pre-register to vote and will automatically be sent a ballot when they officially become eligible to vote
- Residing at the address where they are registering
Absentee ballot applications only need to be filled out by those voters that need a ballot sent to a location other than their normal voter registration mailing address. Missionaries, Students, Snowbirds or any voter residing temporarily in another state or other address within Utah may request to have their ballot mailed to that location on a temporary basis.
Duchesne County makes the UOCAVA voting possible for military and overseas voters, managing voter registration and ballot request status, transmitting and tabulating absentee ballots, and assisting these special voters with the information and tools they need. Eligible UOCAVA voters include:
- Members of the Uniformed Services
- Members of the Merchant Marines
- Eligible family members of the above
- U.S. citizens residing outside the U.S.
Duchesne County registered voters that have moved out of the State of Utah, or no longer want to be registered to vote in Utah can have their name removed from the Official Voters’ Records. A voter who requests their name removed from the Official Voters’ Records will no longer be eligible to vote in the State of Utah unless they re-apply for registration.
Party affiliation pertains to partisan primary elections held in even-numbered years where political parties are nominating candidates to represent their party on the November General Election Ballot. To change your party affiliation, fill out the form and return it to Duchesne Clerk’s Office in person, by mail, or by e-mail.
This form may be used by any registered voter who wishes to request a ballot for a political party that has an open primary and is not their registered political party affiliation.
